– Betty Sinclair You are nursing on the surgical ward of your local hospital and taking care of Betty. 94 years old, Betty Sinclair was living in her family home alone after the death of her husband two years ago. A cleaner comes twice a week and a personal care assistant who provides support for her three mornings a week. Betty was attending church last weekend when a young child ran into her and she fell from a standing height onto her outstretched right arm. Unfortunately, Betty sustained fractures to her radius and ulnar which required orthopaedic intervention in the operating rooms. She is now day four post-operative and you are caring for her on the morning shift. The handover from night duty states that she slept intermittently overnight but had multiple complaints of pain from her surgical site. She was administered analgesia as per her medication chart with good effect. After falling Betty has lost confidence in her mobility and now refuses to mobilise unless she has a nurse or physiotherapist to assist her. Given the workload on the ward this is very difficult for nursing staff and so Betty has had limited mobilisation in the past few days. Betty’s medical & medication history includes… Type 2 Diabetes Mellitus – treated with Metformin & Novorapid Atrial fibrillation – treated with a beta blocker Osteoporosis – treated with denosumab injection Rheumatoid arthritis – treated with long term, lose dose steroid Depression CVA In addition to these she has been receiving Targin 10/5 and Oxycodone 5mg PRN (using 25 – 30mg per day on average) for her acute pain related to her injury.
