5NH024 Skills for Safe and Effective Practice Cohort 119 Assignment brief Module content: During this module, you will develop your knowledge of how to assess, respond and manage changing mental, physical, cognitive and behavioural health states, and respond to distress and deteriorating states of mental, physical, cognitive and behavioural health. Learning Outcomes: LO1:Recognise, respond and justify approaches to distress or deteriorating states of mental, physical, cognitive and behavioural health. LO2:Demonstrate knowledge of a range of interventions including pharmacology and pharmacodynamics in the assessment and management of changing health states. LO3:Demonstrate how an individual’s capacity for independence and self-care might be assessed in a range of diverse settings using a person-centred approach. Assignment brief: Analyse and evaluate the information presented in a case study scenario. Use the information to determine and justify your interventions in relation to safe and effective practice. Guidance / suggested approach to the assignment: Select a patient/client, from the case studies provided. Using the information available, apply a systematic approach (ABCDE) to assess the patient/client’s current health status. Justify the use or application of any additional theories / models / frameworks, and discuss how you would recognise their distress or deterioration. (LO1) Identify 2-3 nursing interventions of your choice that might be used.in assessing or managing the patient/client’s altered health state. (You may include relevant pharmacological interventions to support this.) With reference to relevant supporting literature, provide rationale and justification for your interventions. (LO2) Consider the patient/client’s current health state, their level of independence and potential future healthcare needs. Analyse self-care interventions that are available to the patient/client/family.
